How to Become a Consumer
Whether you're a family or a Waiver Support Coordinator making a referral, getting started is simple. Here's the path from first call to first day.

Reach out. Call (407) 813-2112, email info@mirrahimage.com, or send the referral form below. Case workers can attach the individual's support plan and current authorizations.

Meet & plan. We schedule a no-cost meeting to learn the individual's goals, needs, and daily routine, and confirm which services fit their plan.

Confirm funding. We coordinate with the Waiver Support Coordinator to verify the service authorization and complete enrollment paperwork.

Start services. We match the individual with the right team and set a start date. Families get portal access to follow along from day one.
Funding We Accept
APD iBudget Waiver services, and scholarships through our nonprofit partner Inspire Beyond Limits for families without waiver funding. [Owner to confirm any additional funding accepted.]
Credentials
Florida APD-qualified provider. [Owner to add: APD provider / license number and EIN.]
🎓 Scholarships — Inspire Beyond Limits
No APD waiver? You may still be able to attend. Our nonprofit partner Inspire Beyond Limits provides scholarships so families can access Mirrah Image services without paying us directly. Apply on their website — once you're approved, they send us the details, we set up your account, and you simply log in and choose the services you need.
